Do you need some help while you are in hospital and support to help get things in place for when you return home?

What we do

Whether you are going into hospital or currently in hospital, we can support you and make sure you're able to return home safely.

We work closely with local hospitals and community organisations to make sure you get the right support when leaving hospital. The local hospitals we work with are:

  • Weston General Hospital
  • Southmead Hospital
  • Bristol Royal Infirmary.

That means, if you find yourself at any of these hospitals, we can make sure you get back home safely and quickly.

We receive referrals directly from hospital medical professionals. If you know you'll be going into hospital, or find yourself in hospital, you can also self-refer in advance to prepare for the support you may need. More information about self-referrals is available below.

The service is free and funded by North Somerset Council for people over 18. You don't need to live in be an Alliance Homes customer to access this support.

How we can help

Helping you to live independently

We can assist you with:

  • support to return home following a stay in hospital including pre-planned procedures
  • Make sure your home is warm and safe for you to return to
  • make sure that you have food and other essential supplies following your stay in hospital and if required we can make arrangements to collect essentials items from your home
  • access other health and social care services including meals on wheels, installing key safes and Technology-Enabled Care services, such as Carelink, handy-man services and other community-based services for independent living.

Your health and wellbeing

Find support to keep you healthy, connect with others, access health, social care, and community wellbeing services.

Managing your finances

Get help with rent arrears, money problems, and access to debt advice as well as support to benefits or grants.

Moving home

Get support with housing choices as your needs change, and help you settle into your new home by finding furniture, household items, and setting up utility accounts.

Support for adult carers

Through our Home from Hospital service, we also provide dedicated support for unpaid carers, working closely with our carers support service. We can refer unpaid adult carers to advice, respite services, and a variety of groups and activities to help them manage their caring responsibilities.

If you have a hospital visit coming up, or find yourself admitted unexpectedly, and you need support when you return home, we're here to help. You can complete our self-referral form before or during your hospital stay to get started.
